Forms

There are two types of forms for employees. To assist you with completing these forms, you can refer to Disclosure Scotland’s Guidance Notes below:

If your address details have since changed, please include them on a separate piece of paper

If you have changed your name, please provide documents as proof

Employers

You don't need to complete any part of the application form, only the applicant and Community Pharmacy Scotland complete the form.

Payment

Our payment options are by postal order, cheque, bank transfer or cash. We can't accept any credit/debit card payments currently. If you are paying by bank transfer, please include a sheet of paper with your application form giving the transfer date, amount and the reference name used. Cheques and postal orders should be made payable to ‘Community Pharmacy Scotland’.

Identification

Please don't send original documents with your application, we only need photocopies.

Due to changes by the DVLA we cannot accept the counterpart of your licence as proof of ID, we can only accept the photo-card. The older style of paper licences are still acceptable. Utility bills used to prove your current address should be dated within 6 months of your application.

Change in Personal Details

On the Existing PVG Scheme Member application form, question B7 asks you if there are any changes to your personal details and this includes your address. If your address has changed since your last certificate, please follow the guidance on the form. If you are not sure, please contact Disclosure Scotland's helpline on 03000 2000 40 and select option 3 to find out what address they hold for you.
