Private CD Prescriptions and Requisitions

Any private prescribing of controlled drugs must be done on a specific PPCD(1) form, which can be ordered by prescribers once they have registered with a relevant NHS Health Board.


Similarly, private requisitions of controlled drugs must be written on standardised CDRF forms, with a similar process for the prescriber to follow to obtain the forms. This is all part of controlled drug governance, with the aim being for the NHS to have oversight of all private CD activity.

Our colleagues at Healthcare Improvement Scotland have asked us to pass on a friendly reminder that, whilst copies of these documents should be made and kept on pharmacy premises, the originals should be submitted to NSS along with all of your prescriptions in your monthly/twice monthly bag. Only by doing this can they get a good picture of prescribing behaviours across the country.
